Incident Reporting and Feedback Procedure

The Club’s reporting system is separate to national reporting schemes described by the BGA here:

Certain events have to be reported to the BGA, the Police, the Air Accidents Investigation Branch or the Health and Safety Executive as appropriate.

The Club’s scheme is designed to improve safety by facilitating learning from lesser events that may not require reporting at the national level. If you are unsure about whether an incident needs reporting nationally, then start off by filling in a local incident form and the CFI and Safety Coordinator will decide on what further reporting is appropriate.

Anonymity

You can submit a report anonymously or include your name. Regardless, reports back to the club after an incident has been reviewed will be anonymised as much as possible. The intention is to draw out learnings, rather than point the finger.

Process

Reports should preferably be made on-line via the Members’ website. Alternatively send a private WhatsApp to the Safety Coordinator and they will add it to the system. Paper forms and email reports will also be accepted.

When you fill out the form, it goes by email to the Safety Coordinator, Chairman, and CFI in the first instance. They will read reports as soon as possible after submission and meet as necessary to discuss incidents, assess learnings and propose actions.  The CFI will then consult with instructors and/or tow pilots as needed and the Safety Coordinator will take forward any reporting to members or other bodies as required.

Types of Incidents

Serious incidents/accidents will be reported to the relevant bodies by the Safety Coordinator. A brief summary will appear as soon as practical on the website and a notification emailed to Members.

Less serious events will be appropriately anonymised and summarised, then posted on the Safety section of the website. Members will get an email alert of a new report.

Safety Culture

All Club Members need to be committed to using this system in a transparent and positive way. It is a condition of membership of the club.
